Report on carbapenemase-producing rare sequence types of Escherichia coli and Enterobacter hormaechei

Introduction and Aim: Carbapenem Resistant Enterobacteriaceae (CRE) have emerged at an alarming rate. Multi locus sequence typing (MLST) is important parameter for identifying drug resistant organisms. The present study was carried out elucidating the mechanisms of CRE MLSTs associated with CRE. Materials Methods: (n=14) were obtained from various clinical samples subjected to Rapidec Carba NP (CNP) test multiplex polymerase chain reaction (M-PCR) five isolates proceeded whole genome sequencing (WGS). ?-lactamase (bla) genes analysed using Resfinder CARD tool. Bioinformatics tools: mlplasmids, plasmid finder, mobile element Center Genomic Epidemiology (CGE) toolbox used. Results: All positive CNP bla M-PCR. Isolates (J21, J22, J23, J27) identified as Escherichia coli while (J34) Enterobacter hormaechei. MLST showed E. (J21& J22) ST648; (J23) ST940; (J27) ST 2851, hormaechei closest ST1325. Genes blaTEM, blaNDM & blaampC found be in all isolates; blaCTX-M but not blaOXA (J34); ESBL blaSFO-1 (J34). Conclusion: ESBLs (blaTEM blaCTXM) metallo beta-lactamase -MBL (blaNDM) cause carbapenem resistance rare types while; (blaSFO-1) MBL

We characterized Escherichia coli ST131 isolates among 116 carbapenemase-producing strains. Of isolates from 16 countries collected during 2008-2013, 35% belonged to ST131 and were associated with blaKPC, H30 lineage, and virotype C. This study documents worldwide incidents of resistance to "last resort" antimicrobial drugs among a common pathogen in a successful sequence type.

We report on the first detection of the NDM-1 carbapenemase in Italy, in Escherichia coli isolated in October 2009. Prolonged colonization and relapsing infection by NDM-1-positive E. coli were observed in a patient (index case) with an indirect epidemiological link with areas of endemicity. Transient colonization was apparently observed in another patient linked with the index case.
